4Recruitment Services are seeking a Personal Advisor Leaving Care for our client based in Lincoln. Hybrid working, the client requires 2 days working  from the office.

As a Leaving Care Worker, you will play a vital role in supporting young people aged 16–25 who are transitioning from care to independent living. You will act as a Lead Professional, building strong relationships and delivering tailored support plans that empower care leavers to achieve their goals.

DUTIES & RESPONSIBILITIES INCLUDE:

• Caseload Management: Hold responsibility for a defined caseload of care leavers, ensuring timely assessments, planning, and interventions.
• Pathway Planning: Develop and review aspirational Pathway Plans covering accommodation, education, employment, health, and financial independence.
• Practical Support: Assist with budgeting, bills, tenancy management, shopping, and life-skills training.
• Advocacy & Coordination: Work collaboratively with multi-agency partners (housing, education, health, benefits) to secure positive outcomes.
• Emotional Support: Provide trauma-informed, relationship-based support to promote resilience and wellbeing.
• Safeguarding: Identify and manage risks, ensuring compliance with statutory duties and safeguarding protocols.
• Record Keeping: Maintain accurate case notes and reports in line with local authority standards.
• Duty Cover: Participate in duty rota to respond to urgent needs and crisis situations.

ESSENTIAL REQUIREMENTS INCLUDE:

• Qualifications: NVQ Level 3 in Health & Social Care (or equivalent).
• Experience: Minimum 2 years working with young people in care or leaving care services.
• Knowledge: Children Act 1989, Care Leaver entitlements, safeguarding frameworks.
• IT proficiency (case management systems, MS Office).
• Full UK driving licence and access to a vehicle.
• Enhanced DBS (Children & Adults)
