4Recruitment Services are seeking a Personal Advisor for a Leaving Care Team for our client based in Wrexham

 This is a unique opportunity to support young people who are transitioning out of care, helping them to live independently and successfully reintegrate into the community.

As a Personal Advisor, you will play a vital role in the lives of care leavers aged 15.9 years and older. You’ll be responsible for providing the advice, support, and guidance that young people need to prepare for adulthood and live independently.

DUTIES & RESPONSIBILITIES INCLUDE:

  • Build professional relationships with care leavers aged 15.9+ years and complete Pathway Plans with them by age 16.
  • Provide advice to young people, carers, and staff on benefit entitlements, education, employment, and training opportunities.
  • Attend Pathway Plan Reviews and Looked After Children Reviews to ensure the young person’s needs are met.
  • Work with case-holding social workers to implement and review Pathway Plans for both under 18s and post-18s.
  • Support young people in developing their education packages and individual support programs.
  • Deliver leaving care programs to help young people reintegrate into the community, including occasional evening or weekend work.
  • Maintain productive relationships with social workers, agencies, and organizations to support young people’s needs.
  • Ensure young people have access to necessary services and advocate for their needs, following safeguarding procedures.
  • Assist young people in securing their first tenancy, providing support with budgeting, utilities, and first home grants.
  • Participate in team meetings and case reviews, contributing to the development of team practices.
  • Promote anti-discriminatory practices and ensure confidentiality in line with Departmental policies.
  • Contribute to assessments and attend planning meetings and statutory reviews for those under 18.
  • Take responsibility for personal development, participating in training, supervision, and other developmental programs.
  • Ensure timely benefit applications and liaise with welfare rights services regarding changes in DWP legislation.
  • Develop positive working relationships with care leavers, their families, carers, and partner agencies.

ESSENTIAL REQUIREMENTS INCLUDE:

  • Minimum of 5 Grade "C" GCSE passes (or equivalent), including Maths and English, or equivalent relevant experience demonstrated.
  • NVQ Level 3 in Health and Social Care or equivalent qualification.
  • Good working knowledge of the Children’s Act 1989 and Children (Leaving Care) Act 2000.
  • Experience of working with young people with complex issues
  • Enhanced DBS (Children & Adults)
