About the Role

We are looking for reliable and compassionate Community Care Workers to join our team in Liskeard and the surrounding areas.

This is a temporary full-time opportunity, starting as soon as possible and running through to the end of January 2027.

You will be supporting clients who have recently been discharged from hospital and need additional care and support to regain their confidence, independence and ability to manage day-to-day life at home.

This is a rewarding role where you can make a genuine difference to people during an important stage of their recovery.

Key Responsibilities

As a Community Care Worker, you will:

  • Provide person-centred care and support to clients in their own homes.
  • Support individuals following discharge from hospital.
  • Encourage clients to regain and maintain their independence.
  • Assist with personal care and day-to-day activities where required.
  • Support with medication in line with training and individual care plans.
  • Follow care plans and maintain accurate records.
  • Promote dignity, choice and respect at all times.
  • Follow safeguarding, health & safety and infection control procedures.
  • Work alongside other care professionals and members of the wider support team.

Working Hours

The standard shifts are:

There will also be opportunities to pick up additional outstanding shifts over the coming weeks.

Travel

This role involves travelling between clients across Liskeard and the surrounding areas.

Due to the amount of travel involved:

  • Travel time is paid
  • Full mileage is paid

You will therefore need to be comfortable travelling between different client locations throughout your working day.

About You

We are looking for people who are:

  • Caring, compassionate and patient.
  • Reliable and punctual.
  • Comfortable supporting vulnerable adults.
  • Able to work independently and as part of a team.
  • Flexible and willing to travel between clients.
  • Confident following care plans and procedures.
  • Committed to providing high-quality, person-centred care.

Both male and female applicants are welcome.

Previous care experience is beneficial, but we're also interested in people who have the right attitude, reliability and commitment to helping others.

The following training will be required:

  • Manual Handling
  • Safeguarding
  • Medication
  • First Aid
